Thank you for completing a diet history form and requesting a nutrition consult for your pet.

Please ask your vet to also complete a consult referral form, so that we have your pet’s medical information. We are unable to proceed with a consult without your vet’s form.

Once we receive both the required forms, we will go ahead and book in your pet’s nutrition consult, and email you an invoice if requested by your vet. Our turnaround time is typically 4 weeks.

Please do not expect to hear from us during this time period. If you have not received your report and recipes within 4 weeks, please contact your vet for more information.
